Terms and conditions

1. Description of Services. Beginning on the Effective Date, Provider will provide to the Recipient the following services:

 

Services Provided:
 
Medical Consultation: Initial medical evaluation and assessment.
Monthly Medical Follow-up: Regular check-ins to monitor progress and adjust treatment as necessary.
Nutrition Education Session: Guidance on diet and healthy eating habits.
Prescription of Semaglutide: Starter dose and titration plan over a three-month period, or the purchased treatment period.
 
Terms and Conditions:
 
The Client acknowledges that the Provider is offering a weight loss program that includes the use of the medication Semaglutide.
The Client understands that while Semaglutide has been effective in aiding weight loss for many patients, individual results may vary.
The Provider does not guarantee any specific weight loss outcome.
The success of the weight loss program largely depends on the Client?s personal motivation, discipline, and adherence to the program.
The Client agrees to follow the treatment plan and attend all scheduled appointments.
The Client understands the potential risks and side effects associated with the use of Semaglutide and agrees to promptly report any concerns to the Provider.

 

2. Payment. The Recipient agrees to pay the Provider as described in the service or membership. 

 

Payment Terms and Consequences of Non-Payment:
 
Payment Due Date: The Client agrees to make timely payments for the services rendered by the Provider on the date specified in their individual payment plan. Payment schedules and due dates will be communicated in advance to the Client.
 
Hold on Medication Prescription: In the event of a failure to pay by the due date, the Provider will place a hold on the shipment of the prescribed medication. This hold will remain in effect until full payment is received. The Client will be notified of this hold via phone, email or text.
 
Contract Termination: If payment is not received within 7 days of the due date, this contract will be considered breached and subsequently terminated. Upon termination, all future prescriptions planned under this agreement will be discontinued. The Client will be notified of impending termination and given a chance to rectify the situation.
 
Reinstatement: Reinstatement of services and prescriptions after termination will require a new agreement and may be subject to additional terms and conditions, including potential reinstatement fees, at the discretion of the Provider.

 

In addition to any other right or remedy provided by law, if the Recipient fails to pay for the Services when due, the Provider has the option to treat such failure to pay as a material breach of this Contract and may cancel this Contract and/or seek legal remedies.

 

3. Term. This Contract will remain in effect for a period of 3 months, 6 months or 12 months depending on chosen plan.

 

4. Confidentiality. The Provider and its employees, agents, or representatives will not at any time or in any manner, either directly or indirectly, use for the personal benefit of the Provider or divulge, disclose, or communicate in any manner any information that is proprietary to the Recipient. The Provider and its employees, agents, and representatives will protect such information and treat it as strictly confidential. This provision will continue to be effective after the termination of this Contract. Any oral or written waiver by the Recipient of these confidentiality obligations which allows the Provider to disclose the Recipient's confidential information to a third party will be limited to a single occurrence tied to the specific information disclosed to the specific third party, and the confidentiality clause will continue to be in effect for all other occurrences.

 

5. Default. The occurrence of any of the following shall constitute a material default under this Contract:

 

       

 (a)The failure to make a required payment when due.

   

       

 (b)The insolvency or bankruptcy of either party.

   

       

 (c)The subjection of any of either party's property to any levy, seizure, general assignment for the benefit of creditors, application, or sale for or by any creditor or government agency.

   

       

 (d)The failure to make available or deliver the Services in the time and manner provided for in this Contract.
